Methodology — Cohorts are derived purely from the FI/QI gates, not sized against intervention effort.
Dual-solved clears both floors; FI-only blocker clears QI but needs a margin push;
QI-only blocker clears FI but needs a quality push; both blockers miss both gates.
Quick wins are facilities within 0.05 of both floors — the 60-day push list.
Important: dual-mandate clearance (FI ≥ 0.50, QI ≥ 0.50) is a different threshold from
"Solved" band on CAHSP (≥ 85); a facility can clear the mandate gates and still be in the High or Moderate band
